Wise Options

Wise Options helps older adults and families find the resources to meet the challenges of aging using a certified geriatric care manager. Our help can ease stress and bring clarity to what lies ahead. We can:

  • Conduct in-depth assessments of medical, psychosocial and environmental factors
  • Develop a customized care plan to meet individual needs and maintain quality of life
  • Coordinate with health care providers and social services
  • Communicate with families near and afar, and mediate if necessary
  • Advocate for client care with providers
  • Address needs that arise during living transitions — from hospital to rehab to home, or from home to a supportive facility
  • Evaluate potential resources, including government and veteran benefits
  • Provide guardianship support services
  • Member of National Association of Professional Geriatric Care Managers and a National Certified Guardian.

Individual, marital, and family counseling help clients cope constructively with challenges.

Case Management

We provide food and emergency support to families in need through Project Manna and help them connect with other resources and secure benefits. This includes linking individual or family to other agencies such as mental health or disability related services.

Staying Connecting

Founded in 2002, Staying Connected uses trained, supervised volunteers to transport older adults to essential appointments including:

  • Medical appointments
  • Grocery shopping
  • Banking
  • Lawyers’ visits
  • Tax appointments
  • Medicare counseling appointments
